Hi all–Kristin from Breakthrough T1D Community Engagement here. We wanted to share some updated information about another 501(c)(3) nonprofit serving the T1D community, Blue Circle Health. (You may have seen information about them originally posted about a year ago). Now available to adults living with T1D in Florida, Maine, and Vermont, this organization provides virtual care, education, and support at no cost to them. The program’s whole-person approach includes medical care, diabetes education, peer coaching, diabetes supportive counseling, insurance navigation, supplies and medication, and referrals to community resources that can help with access to housing, employment and food.
Because T1D is a 24/7 condition that is largely self-managed, Blue Circle Health educates and empowers people with T1D to ensure they have the skills, tools and confidence to manage their condition. The program, which typically lasts 6 months, is currently available in Florida, Maine, and Vermont to adults 18+ who speak English or Spanish.
